In the previous article Pressure therapy for burn scars: What are the considerations for scar stability? Options in relation to scar stability requirements, we discussed different types of options for pressure therapy for burn scars and their characteristics in relation to scar stability requirements. In this second article, we’ll discuss how to choose among different interventions for pressure therapy for burn scars when scar stability is not optimal.


How to select an intervention for pressure therapy for burn scars depending on scar stability?

Based on the characteristics of different pressure treatment options and considering the condition of scar stability, the location of scars and the pressure therapy needs, we organized in Picture 2 a “thinking process” tool that therapists can use to evaluate and select the best option.

Key points about selection pressure therapy options based on scar stability

When the scar is unstable:

  • Use self-adherent elastic bandages if the scar is on the hands or feet.

  • Use elastic bandage if the scar is on the trunk or upper/lower limbs.

 

If long-term scar control is necessary:

  • Use pressure garments when scar stability has increased.

 

If there are areas that cannot receive enough pressure (like flat or concave areas):

  • Use silicone gel sheets, Otoform inserts or pressure inserts made with other types of materials (ex: inserts made with jeans fabric) to strengthen areas with insufficient pressure, but be sure to evaluate the stability of the scar.
